Processes are described through interviews, local diagrams and tacit knowledge; there is no governed end-to-end representation. This becomes most visible when decisions must cross functions, systems or organisational boundaries and no single owner can supply a trusted answer.
The documented process is an opinion rather than an operational truth. The underlying weakness is the absence of a governed, connected and queryable representation of the operating model.
Improvement, automation, audit and AI initiatives are built on false assumptions; failure is scaled rather than removed. At enterprise scale, each new change, audit, incident or transformation programme rebuilds the same knowledge at additional cost.
A shared process system of record, validated through analysis, allows decisions to begin from an agreed baseline. In practical terms, leaders and delivery teams can work from the same baseline, interrogate relationships quickly and govern updates as the organisation changes.
